We report the experimental determination of the magnetic exchange parameter (J/kB= 2.88 ± 0.02 K) for the Spin-3/2 ferromagnetic (FM) Kagomé lattice system: Co3V2O8using the temperature dependence of dc-magnetic susceptibilityχ(T) data by employing the fundamental Heisenberg linear chain model. Our results are quite consistent with the theoretically reported nearest neighbor dominant FM exchange coupling strengthJex-NN∼2.45 K. Five different magnetic phase transitions (6.2-11.2 K) and spin-flip transitions (9.6-7.7 kOe) have been probed using the∂(χT)/∂Tvs.T, heat capacity (CP-T) and differential isothermal magnetization curves. Among such sequence of transitions, the prominent ones being incommensurate antiferromagnetic (AFM) state at 11.2 K, commensurate AFM state at 8.8 K, and commensurate FM state across 6.2 K. All the successive magnetic phase transitions have been mapped onto a single H-T plane through which one can easily distinguish the above-mentioned different phases. The magnetic contribution of theCP-TnearTN(11.2 K) has been analyzed using the power-law expressionCM=A|T-TN|-αresulting in the critical exponentα= 0.18 ± 0.01 (0.15 ± 0.003) forTTN), respectively for the Co3V2O8. It is interesting to note that non-Debye type dipole relaxation is quite prominent in Co3V2O8and was evident from the Kohlrausch-Williams-Watts analysis of complex modulus and impedance spectra (0⩽ß⩽1). Mott's variable-range hopping of charge carriers process is evident through the resistivity analysis (ρac-T-1/4) in the temperature range 275 ∘C-350 ∘C. Moreover, the frequency-dependent analysis ofσac(ω) follows Jonscher's power law yielding two distinct activation energies (Ea∼0.37 and 2.29 eV) between the temperature range 39 ∘C-99 ∘C and 240 ∘C-321 ∘C.

The present study describes four whole genome sequences of WSSV obtained from coastal regions of Bangladesh.

Pigeon pea (Cajanus cajan) is widely cultivated for its nutritional and medicinal value yet remains an orphan crop as productivity has not been improved because of a lack of genome and non-coding genome information. Non-coding RNAs, like miRNAs and long non-coding RNAs (lncRNAs), are involved in regulation of growth, metabolism, development, and stress response, and have a critical role in post-transcriptional gene regulation (PTGR). We attempted to elucidate the roles of miRNAs and lncRNAs in pigeon pea through experimental validation of computationally predicted miRNAs and lncRNAs and targets of miRNAs on mRNAs. We experimentally validated 20 miRNAs and 11 lncRNAs. We predicted cleavage sites of three miRNA targets: serine/threonine-protein kinase, polygalacturonase, beta-galactosidase. We identified 469 targets of 265 miRNAs and their functional annotations using computational methods. We built a miRNA-mRNA-lncRNA network model, with the miRNAs targeting both mRNAs and lncRNAs, to obtain information on the interplay of these three molecules. A confirmed interaction through experimental validation was established between miRNA, namely cca-miR1535a targeting the mRNA for beta-galactosidase, as well as the lncRNA cca-lnc-020033. Our findings increase knowledge of the non-coding genome of pigeon pea and their roles in PTGR and in improving agronomic traits of this pulse crop.

Deep learning methods, a powerful form of artificial intelligence, have been applied in a number of spectroscopy and gas sensing applications. However, the speciation of multi-component gas mixtures from infrared (IR) absorption spectra using deep learning remains to be explored. Here, we propose a one-dimensional deep convolutional neural network gas classification model for the identification of small molecules of interest based on IR absorption spectra in flexible user-defined frequency ranges. The molecules considered include ten that are of interest in the atmosphere or in industrial and environmental processes: water vapor, carbon dioxide, ozone, nitrous oxide, carbon monoxide, methane, nitric oxide, sulfur dioxide, nitrogen dioxide, and ammonia. A simulated dataset of IR absorption spectra for mixtures of these molecules diluted in air was generated and used to train a deep learning model. The model was tested against simulated spectra containing noise and was found to provide speciation predictions with accuracy from 82 to 97%. The internal operation of the model was investigated using class activation maps that illustrate how the model prioritizes spectral information for classification. Finally, the model was demonstrated for the prediction of speciation for two synthetic experimental mixture spectra. The proposed model and the dataset generation strategies are generalized and can be implemented for other gases, different frequency ranges, and spectroscopy types. The multi-component speciation method developed herein is the first application of a convolutional neural network model, trained on HITRAN-based simulations, for spectral identification.

The increasing cost of milk production, in association with tighter manure N application regulations and challenges associated with ammonia emissions in many countries has increased interest in feeding lower crude protein (CP) diets based on legume silages. Most studies have focused on alfalfa silage, and there is a lack of information on low CP diets based on red clover silage. Our objectives were to examine the effects of dietary CP content and supplementing a low CP diet with dietary starch or rumen-protected Met (RPMet) on the performance, metabolism, and nitrogen-use-efficiency (NUE; milk N output/N intake) in dairy cows fed a red clover and grass silage-based diet. Fifty-six Holstein-Friesian dairy cows were blocked and randomly allocated to 1 of 4 diets over a 14-week feeding period. Diets were based on red clover and grass silages at a ratio of 50:50 (dry matter (DM) basis) and were fed as a total mixed ration, with a forage-to-concentrate ratio of 53:47 (DM basis). The diets were formulated to supply a similar metabolizable-protein (MP) content, and have a CP concentration of 175 g/kg DM (CON), or 150 g/kg DM (LP = low protein), or LP supplemented with additional barley as a source of starch (+ 64 g/kg DM; LPS) or RPMet (+ 0.3 g/100 g MP; LPM). At the end of the 14-week feeding period, 20 cows (5 per treatment) continued to be fed the same diets for a further 6 d, and total urine output and fecal samples collected. We observed that dietary treatment did not affect DM intake, with a mean of 21.5 kg/d, however, there was an interaction between diet and week with intake being highest in cows fed LPS in wk 4 and CON in wk 9 and 14. Mean milk yield, 4% fat corrected milk and energy corrected milk were not altered by treatment. Similarly, we found no effect of dietary treatment on milk fat, protein or lactose content. In contrast, milk and plasma urea concentrations were highest in cows fed CON. The concentration of blood plasma ß-hydroxybutyrate was highest in cows receiving LPM and lowest in LPS. Apparent NUE was 28.6% in cows fed CON, and was higher in cows fed any of the low protein diets (LP, LPS or LPM) with a mean value of 34.2%. The sum of milk fatty acids with a chain length below C16:0 was also highest in cows fed CON. We observed that dietary treatment did not affect the apparent whole-tract nutrient digestibility of organic matter, N, neutral detergent fiber and acid detergent fiber, with mean values of 0.785, 0.659, 0.660 and 0.651 kg/kg respectively, but urinary N excretion was approximately 60 g/d lower in cows fed the low CP diets compared with CON. We conclude that reducing the CP content of red clover and grass silage-based diets from 175 to 150 g/kg DM while maintaining MP supply did not affect performance, but reduced the urinary N excretion and improved NUE, and that supplementing additional starch or RPMet had little further effect.

Patients with chronic kidney disease (CKD) are more likely to have complications due to cardiovascular diseases (CVD). This cross sectional descriptive study was conducted in the Department of Medicine and Nephrology, Chittagong Medical College (CMC), Chittagong, Bangladesh from June to December, 2014 and was performed to estimate the risk of cardiovascular events according to Framingham's Risk Score of CKD patients. In this research, 100 patients who was diagnosed as CKD and admitted in the Chittagong Medical College Hospital were enrolled. Estimated glomerular filtration rate (eGFR) was calculated with the MDRD formula. Cardiovascular risk factors were analyzed by Framinghams criteria and after compilation data were analyzed by SPSS-18.0. Among 100 patients, most of patients were under 60 years of age where males (56.0%) were more than females (44.0%). Framingham Cardiovascular Risk Score revealed that most of the male were at high risk group compared to female. Regarding different components of lipid profile with Framingham risk score, higher risk group had high mean total cholesterol (212.17mg/dl) and also low mean HDL level (38.58mg/dl). Patient with different stages of CKD with cardiovascular risk showed stage 5 CKD was 80%, stage 4 was 15.0% and rest 5 was at stage 3 and most were in low risk group (56.0%). There were significant positive correlations found in between age and serum total cholesterol level with the cardiovascular risk scoring but not with HDL and eGFR. The predominant cardiovascular risk factors were age, male gender, smoking and high serum total cholesterol.

BACKGROUND: The host-microbe interactions are complex, dynamic and context-dependent. In this regard, migratory fish species like hilsa shad (Tenualosa ilisha), which migrates from seawater to freshwater for spawning, provides a unique system for investigating the microbiome under an additional change in fish's habitat. This work was undertaken to detect taxonomic variation of microbiome and their function in the migration of hilsa. METHODS AND RESULTS: The study employed 16S rRNA amplicon-based metagenomic analysis to scrutinize bacterial diversity in hilsa gut, skin mucus and water. Thus, a total of 284 operational taxonomic units (OTUs), 9 phyla, 35 orders and 121 genera were identified in all samples. More than 60% of the identified bacteria were Proteobacteria with modest abundance (> 5%) of Firmicutes, Bacteroidetes and Actinobacteria. Leucobacter in gut and Serratia in skin mucus were the core bacterial genera, while Acinetobacter, Pseudomonas and Psychrobacter exhibited differential compositions in gut, skin mucus and water. CONCLUSIONS: Representative fresh-, brackish- and seawater samples of hilsa habitats were primarily composed of Vibrio, Serratia and Psychrobacter, and their diversity in seawater was significantly higher (P < 0.05) than freshwater. Overall, salinity and water microbiota had an influence on the microbial composition of hilsa shad, contributing to host metabolism and adaptation processes. This pioneer exploration of hilsa gut and skin mucus bacteria across habitats will advance our insights into microbiome assembly in migratory fish populations.

Zinc Oxide (ZnO) nanoparticles (NPs) have been synthesized by a simple chemical precipitation method. The effect of monoethanolamine (MEA) content in different solvents on ZnO NPs synthesis and their structural properties has been investigated. The NPs synthesized by using isopropanol (IPA) with 15 ml MEA as a stabilizer under the most favorable conditions (deposition time, td = 120 min, temperature = 60 °C) showed good structural properties. Synthesized NPs exhibited beneficial structural properties after annealing. The hexagonal wurtzite crystal structure of ZnO NPs was verified by XRD. Different models were used to calculate structural parameters such as crystallite size, strain, stress, and energy density for all the reflection peaks of XRD corresponding to ZnO lying in the range 2θ = 15°-80°. The crystallite size of the ZnO nanoparticles was found to be 50-60 nm. FTIR and EDX confirmed the presence of ZnO NPs in the samples. SEM micrograph of all the samples revealed that the grain sizes decrease gradually with the increase of the amount of MEA. UV-Visible diffuse reflectance spectroscopy results provide evidence that the ZnO NPs possess broader absorption bands, together with high band gap energy. The ZnO NPs synthesized with IPA solvent have the highest transmittance and band gap energy of 3.3eV. According to DLS data, various content of MEA stabilizer in solvent affects the hydrodynamic size of ZnO NPs.

The experiment was conducted at the research field, Department of Agronomy, Hajee Mohammad Danesh Science and Technology University, Dinajpur from December 2017 to May 2018 to find out the best treatment of foliar application of urea on the growth and yield of boro rice cv. BRRI dhan28. The experiment consisted of 10 treatments, laid out in a randomized complete block design in triplicate. The recommended doses (RD) of urea, TSP, MOP, gypsum, ZnSO4, and borax were applied during land preparation except for urea at 250, 75, 100, 75, 7, and 5 kg ha-1, respectively, where urea was applied as per treatment specification. The results revealed that the application of N fertilizer as foliage along with soil significantly influenced the growth, plant characteristics, and yield of BRRI dhan28. There was no significant difference between T8 (70% in soil and 10% as foliage) and T9 (100% in soil) treatment regarding the maximum panicle length (21.43 and 20.71 cm), fertile grains (117.40 and 113.30), total grains (134.40 and 130.97), 1000-grain weight (24.56 and 23.56 g), grain yield (5.91 and 5.74 t ha-1), straw yield (7.83 and 7.92 t ha-1), biological yield (13.74 and 13.66 t ha-1), and harvest index (43.01 and 42.02%), respectively, in this study. These results indicated that N fertilization as direct soil application (70%) and as foliage application (10%), i.e., 80% N fertilization, produced the highest grain yield and major yield traits which we received by 100% N fertilization as soil that was practiced traditionally by the farmers. The effect of overfertilization (T10) was not positive, producing the highest number of noneffective tillers and sterile grains (nonfilled grains). Therefore, it is possible to achieve an equivalent or more yield by saving 20% urea by the combination of soil (70%) and foliage (10%) application as compared to the traditional method of fertilizer application (100% in soil).

Technology and its continuous advancement facilitate human beings to get rid of their criticality and limitation. Applied artificial intelligence (AAI) is one of the latest forms that delimited the limitation of human beings. Smartwatch acts as an applied artificial intelligence to assist various patients to check medical care without going to hospital and physicians. This (three) multiple-study research focused on the intention to use, purchase, and their satisfaction and spread positive word of mouth among others in the Ghanaian. To investigate these issues two renowned theories were underpinned- TAM theory and the Stimulus-Organism-Response (S-O-R). Total 550, 320, and 170 respondents were interviewed with Google forms due to COVID-19 using social media. AI-enabled smartwatch considering Perceived Ease of Use (PEOU), Perceived Usefulness (PU), Perceived Credibility (PC), Perceived Self-Efficacy (PSE), and Perceived Financial Cost (PFC) were significant on intention to adoption and adoption intention on actual purchase. The final study showed device quality, its service level, their usage experience, perceived value, and the extent to which the satisfied customers made positive word of mouth to their friends and family, colleagues and followers. This research is significant in understanding the usage of AI-enabled smartwatches as a device doctor or electronic doctor (e-doctor).

Atopic Dermatitis (AD) is a chronically relapsing, highly pruritic, allergic inflammatory skin disease with significant cost and morbidity to the patients and their families. The underlying cause of AD has not been understood, however some studies have shown initial epidermal barrier defect with subsequent immune activation as the underlying mechanism of AD. Vitamin D is now recognized as an immunomodulator. The role played by vitamin D in atopic dermatitis is controversial and has been the focus of many studies. The aim of the study was to measure serum vitamin D in the form of 25-hydroxy vitamin D in patients with AD and to correlate them with disease severity. This cross-sectional study included 41 patients (25 males and 16 females) of any age with the clinical diagnosis of AD seen in Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from September 2015 to February 2017. Disease severity was determined using Scoring Atopic Dermatitis (SCORAD) index and the patients were divided into three groups: mild (SCORAD index <25), moderate (25-50) and severe (>50). Serum vitamin D levels were classified as sufficient (≥30ng/mL), insufficient (21-29ng/mL) and deficient (≤20ng/mL). Statistical analysis was performed using analysis of variance (ANOVA) and Pearson's correlation coefficient test. P value of <0.05 was considered as statistically significant. Among 41 patients 33 represent infantile and childhood AD and only 8 represent adolescent and adult AD. According to SCORAD index, 12 patients had mild, 20 had moderate and 9 had severe Atopic dermatitis. Levels of 25-hydroxyvitamin D were deficient or insufficient in 75.6% of patients and normal in 24.4% patients. There was no significant association between serum level of vitamin D and the severity of AD (r=-0.173). The mean±SD serum vitamin D level in mild AD (25.7±8.1) was higher compared with those with moderate (23.9±8.8) or severe (19.5±8.3) AD. But the result was not statistically significant (p=0.249). Variables such as sex, age, skin prototype, season and food allergy were not significantly associated with vitamin D levels. The results from this study suggesting that millions of children living in Bangladesh may have suboptimal levels of vitamin D, which should be a matter of public health concern. But these deficient results are not significantly related to AD severity. Thus, the study provides epidemiological evidence against the association of vitamin D status with atopic dermatitis for the first time in Bangladesh.

RESUMEN

A 10 years old boy who was initially diagnosed as choledochal cyst underwent laparotomy. There was presence of necrotic and soft tissue growth within common bile duct (CBD). After extensive toileting of bile duct, a T-tube was placed. His histopathology followed by immunohistochemistry revealed Embryonal Rhabdomyosarcoma. Later the patient received VAC regime chemotherapy. On follow-up imaging, there was no tumour mass in CBD. So, T-tube removed and now the patient is doing well.

BACKGROUND: Acute febrile illness (AFI) is a prevalent disease in developing countries that is difficult to diagnose due to the diversity of infectious organisms and the poor quality of clinical diagnosis. TaqMan array card (TAC) can detect up to 35 AFI-associated organisms in 1.5 h, addressing diagnostic demands. In this study, we aimed to evaluate the role of TAC in determining the causative organisms in hospitalized AFI patients. METHODS: The study had a cross-sectional design and enrolled 120 admitted patients with persistent fever for three or more days from the medicine ward of Chittagong Medical College Hospital (CMCH) and Bangladesh Institute of Tropical and Infectious Diseases Hospital (BITID). Blood samples were collected and then subjected to automated BacT/Alert blood culture, microbial culture, TAC assay, and typhoid/paratyphoid test. RESULTS: The total number of study participants was 120, among them 48 (40%) samples showed a positive result in TAC card, 29 (24.16%) were TP positive and nine (7.51%) were culture positive. The number of organisms detected by the TAC card was 13 bacteria, three viruses, one protozoan, and one fungus. The sensitivity and specificity of the TAC assay for different bacterial pathogen compared to blood culture was 44.44%, and 90.99%, respectively. In contrast, the TP test had a sensitivity and specificity of 100% and 80%, respectively, compared to the blood culture test. CONCLUSION: TAC can be a handful tool for detecting multiple organisms in AFI with high specificity which can facilitate early diagnosis of different pathogens contributing to AFI.

Radial artery occlusion (RAO) is now a major concern in transradial approach (TRA). RAO limits future radial artery use for further TRA, for as a conduit during CABG, for invasive hemodynamic monitoring and for creation of arteriovenous fistula for hemodialysis in Chronic Kidney Disease (CKD) patients through same vascular approach. The effect of duration of hemostatic compression of RAO is unknown in Bangladesh. This prospective observational study was conducted in the department of Cardiology, National Institute of Cardiovascular Diseases, Dhaka, Bangladesh (NICVD) from September 2018 to August 2019, to evaluate the effect of duration of hemostatic compression on the incidence of radial artery occlusion (RAO) after transradial percutaneous coronary intervention. A total of 140 patients underwent percutaneous coronary intervention (PCI) through TRA. RAO was defined as an absence of antegrade flow or monophasic flow or invert flow on Duplex study. In this study 70 patients (Group I) received 2 hours hemostatic compression after transradial PCI. Another 70 patients (Group II) received 6 hours hemostatic compression after transradial PCI. Radial arterial blood flow assessed at early (24 hours) and late (30 days) by color duplex study after the procedure in both groups. Early radial artery occlusion occurred in 4.3% of patients in Group I and 12.8% of patients in Group II, (p=0.04). Late radial artery occlusion occurred in 2.8% of patients in Group I and 11.4% of patients in Group II, the difference was statistically significant (p=0.04). From multivariate logistic regression analysis duration of hemostatic compression time 6 hours (p=0.01), post-procedural nitroglycerine use (p=0.03) and procedure time (p=0.03) were predictors of RAO. Shorter duration of hemostatic compression is associated with a lower incidence of early and late radial artery occlusion after transradial intervention.

The goal of this study was to analyze the efficacy of the Ommaya reservoir within all the different types of hydrocephalus in pediatric patients. At the same time, it's safe for repeated aspirations or long-term retention of the reservoir in the body. This retrospective, cross-sectional study was performed from January 2019 to December 2021, 33 consecutive cases of reservoir implantation were taken into the study irrespective of the etiology of hydrocephalus in the Neurosurgery Department of Bangladesh Medical College Hospital, Dhaka, Bangladesh. These were mostly placed along with endoscopic third ventriculostomy and some were placed as an intermediary procedure to combat shunt complications in emaciated infants. Cerebrospinal fluid (CSF) aspiration was done in case of failed endoscopic third ventriculostomy and the frequency of aspiration depended upon the production of cerebrospinal fluid. Acetazolamide was routinely administered in each patient to reduce the frequency of aspiration. Most of the patients required ventriculo-peritoneal (VP) shunt while they had sufficient body weight and few required no surgery. The average age at presentation was 76.88 days. All the neonates and infants had less weight in terms of their age. 42.4% of babies needed aspiration 2 times per week. Among all cases, 9.1% developed reservoir complications. Complications were not related to the number and volume of aspiration or duration of the reservoir in the body. Two (2) patients died after one year of reservoir implantation due to unknown etiology. Out of the 31 survivors, 3 patients did not need any further aspiration and 19 patients needed a ventriculo-peritoneal shunt, but the reservoir was kept in situ for a future emergency. The rest of them is waiting for a definitive shunt procedure. Other findings include low socioeconomic group was more prone to low birth weight and they carried the burden of congenital hydrocephalus and meningomyelocele. Most affected babies had their prenatal period in arsenic-affected areas in Bangladesh. Overall folic acid supplementation was started after the formation of the neural tube irrespective of socioeconomic status. Ommaya reservoir placement along with endoscopic third ventriculostomy plays a vital role in delaying shunt in endoscopic third ventriculostomy failure. It is a 'time buying' procedure until the baby has sufficient weight for successful shunt surgery. It has been found very effective intermediary intervention for managing shunt infection and it also helps revive a channel in shunt obstruction.

Accurate tracking involuntary head movements is fairly a challenging problem in MR imaging of the brain. Though there are few techniques available to monitor the head movement of the subject for a prospective motion correction, it is still an unsolved problem in MRI. In this theoretical study, we aim to describe an analytical investigation to track head movement inside an MR scanner by calculating the change in induced voltage in the head-mounted coils during the execution of time-varying gradients. We derive an expression to calculate the change in induced voltage in a coil placed in a time-varying gradient. We also derive a general equation to investigate the changes in the induced voltage in a set of coils mounted onto the head for the planar position and orientation of the coils. Each coil is considered as a magnetic dipole with location and sensitivity vectors. The changes of the vectors can track the head movement in the MR scanner by measuring the changes in the induced voltage in the coils. The dipole concept is valid for a wide range of coils. The changes in induced voltage in the coils are linear due to small changes in pose of the head. Movement parameters are estimated from the induced voltage changes. If the random noise voltage is less than 100 µV, it does not significantly affect movement parameters because the change in induced voltage in the coils dominates over the small noise voltage. This method and array of the coils may provide a real-life solution to the long-standing problem of head motion during MRI.

The identification of gas mixture speciation from a complex multicomponent absorption spectrum is a problem in gas sensing that can be addressed using machine-learning approaches. Here, we report on a deep convolutional neural network for multigas classification using terahertz (THz) absorption spectra, THz spectra mixture classifier network or TSMC-Net. TSMC-Net has been developed to identify eight volatile organic compounds in mixtures based on their fingerprint rotational absorption spectra in the 220-330 GHz frequency range. A data set consisting of simulated absorption spectra for randomly generated mixtures, with absorption greater than thresholds representing detectable limits and annotated with multiple labels, was prepared for model development. The supervised multilabel classification problem, i.e., the identification of individual gases in a mixture, is converted to a supervised multiclass classification problem via label powerset conversion. The trained model is validated and tested against simulated spectra for gas mixtures, with and without white Gaussian noise. The trained model exhibits high precision, recall, and accuracy for each pure compound. Class activation maps illustrate the complex decision-making process of the model and highlight relevant frequency regions that are needed to identify unique mixtures. Finally, the model was demonstrated against measured THz absorption spectra for pure species and mixtures, acquired using a microelectronics-based THz absorption spectrometer. The data set generation strategy and deep convolutional neural network approach are generalized and can be extrapolated to other spectroscopy types, frequency ranges, and sensors.

Control of pain in patients with chronic pancreatitis is difficult because 30.0% to 50.0% of patients still experience persistence or recurrence of pain even after surgery. So a combined approach of surgery and coeliac plexus neurolysis was carried out in this study to see the relief of pain and reduce the requirement of analgesics in these patients. This prospective observational comparative study was carried out in the Department of Hepatobiliary, Pancreatic and Liver Transplant Surgery in Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh from November 2017 to October 2018. Forty one (41) study participants with the diagnosis of chronic pancreatitis were included consecutively in this study. The participants were divided into two groups. Group I (n=18) underwent pancreatic surgery with coeliac plexus neurolysis by infiltration of 20ml of 100% alcohol in the loose areolar tissue 10ml each into right and left para-aortic space at the level of coeliac trunk and Group II (n=23) underwent pancreatic surgery only. Participants' preoperative data were collected from patient record file. Number, frequency and intensity of pain and requirement of amount of analgesics for the last 3 months were recorded from patients' history. The intensity of pain was categorized by visual analog scale (VAS) preoperatively. The participants of both groups were followed up at 1, 2 and 3 months interval and asked for disappearance or reduction of pain, frequency of attack and requirement of analgesics. Again visual analog scale was used for categorization of pain. Pain free period was recorded after the end of follow up period. Pain reduction occurred after surgery in both groups. But when pain relief was compared on the basis of VAS (Visual Analogue Scale), it was significantly better in Group I after 1 month of surgery than Group II (p=0.05). But 2 and 3 months after surgery this difference became insignificant (p=0.246 and 0.264). No statistical difference was found in terms of analgesic usage, severe acute attack or hospital admission (p=0.511, 0.439 and 0.495) at the end of 3 months follow up. Participants in Group I had significantly longer pain free period than Group II (p=0.025). Regarding complications, postural hypotension developed in 5.6% (1) patients. Diarrhea developed in 11.1% (2) patients in Group I and wound infection developed in 2 patients in each groups respectively. No patients developed any major complications like anastomotic leakage, deep or organ or space infection. Intraoperative coeliac plexus neurolysis reduces pain immediately after surgery and provides longer pain free period in patients with chronic pancreatitis after surgery.

Acute myocardial infarction (AMI) in younger adults (≤40 years) is being increasingly encountered in recent years among the South Asian population. Data regarding the presentation, risk factors and angiographic findings on this important subset of patients is lacking in our country. The aim of this study was to compare the risk factors and pattern of Coronary artery involvement in younger patients presenting with AMI with that of the older age group. This was a cross-sectional observational study conducted during the period from October 2018 to June 2019. Seventy consecutive AMI patients age ≤40 years and another 70 consecutive AMI patients age >40 years undergoing Coronary Angiogram (CAG) were included in the study. After taking informed written consent; demographic, anthropometric, risk factors, CAG findings were recorded in a pre-designed case record form. The severity of Coronary Artery Disease (CAD) was calculated by using Gensini score. The mean age of the younger and older patient groups was 36.89±4.4 years and 57.00±8.4 years respectively. Among the risk factors, smoking (67.1% versus 45.7%, p=0.017), positive family history CAD (38.6% versus 22.9%, p=0.040) and obesity (34.3% versus 20.0%, p= 0.05) were more common in younger group. Whereas, Hypertension (41.4% versus 72.9%, p=0.010) and DM (28.6% versus 50.0%, p=0.024) were more common in older patients. Younger patients mainly presented with STEMI (60.0% versus 48.6%) and predominantly had single vessel disease (42.9%), whereas older patients readily presented with NSTEMI (51.4%) and had a higher incidence of double vessel disease (32.9%) and triple vessel disease (30.0%). The Median Gensini score was significantly higher among the older patients than in the younger age group. Patients in younger age group showed a different pattern of risk factors and coronary artery involvement in comparison to the older age group. Thus, offering younger individuals to make them aware of these risk factors and their early detection, as well as an effective intervention may help to prevent AMI in younger people.

Diagnosis of primary sclerosing cholangitis (PSC) is often very difficult and may have a suspicion with altered liver functions. PSC is known to be associated with inflammatory bowel disease. This article presents a case study of a 70 years old male patient who presented with obstructive jaundice with recurrent episode of cholangitis in June 2019 at Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh. Evaluation revealed beaded appearance in MRCP with positive relevant markers which raised suspicion of PSC and it was confirmed by biopsy and histopathology of the affected segment in biliary tree. The importance of early detection of primary sclerosing cholangitis in an effort to decrease the morbidity and mortality from cholangiocarcinoma will also be emphasized and our management according to local protocol and outcome of this patient.
